A company is looking for a Primary Care Provider, Nurse
Practitioner / Physician Assistant.
Key Responsibilities
Provide comprehensive, community-based care to vulnerable
seniors
Build long-term relationships with patients in a value-based care
model
Conduct home visits and manage a remote on-call schedule
Required Qualifications
Licensed Nurse Practitioner or Physician Assistant
At least two years of experience working with the elderly
Prescriptive authority and DEA registration
Eligibility to serve Medicare/Medi-Cal participants
