Job Description:
MindPath Care Centers, a part of Community Psychiatry, is
seeking a dedicated and compassionate Outpatient Psychiatric Mental
Health Nurse Practitioner (PMHNP) to join our team. Our center
focuses on outpatient mental health services, providing
personalized therapy and medication management dedicated to
improving mental health. The PMHNP will play a critical role in
assessing psychiatric patients, formulating treatment plans, and
managing patient care to ensure the highest quality of mental
health services. This role requires a blending of clinical
expertise in the field of psychiatric nursing with a passionate
approach to patient-centered care. Assess patient’s mental health
conditions and needs through interviews, observations, tests and
medical history reviews. Develop individualized treatment plans
based on patient needs and conditions. Prescribe medication and
conduct regular follow-ups to manage side effects and adjust
prescriptions as necessary. Provide psychotherapy interventions as
appropriate. Collaborate with healthcare providers, social workers,
and other professionals to discuss treatment plans and progress.
Educate patients and their families about mental health conditions
and maintenance strategies. Maintain complete and detailed patient
records as per the latest health laws and ethics. Proficiency in
psychiatric diagnosis and treatment. Strong skills in interpersonal
communication and patient care. Ability to manage time and
prioritize tasks effectively. Experience in psychotherapy and
counseling. Solid understanding of confidentiality and health care
privacy policies. Medical Insurance Dental Insurance Vision
Insurance Life Insurance 401k Valid certification as a
Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC). Current
and unrestricted Registered Nurse (RN) and APRN license in the
state of employment. DEA certification.
