This role is essential for providing high-quality, patient-centered care to individuals in their homes. Key Responsibilities Conduct comprehensive assessments of patients in their home environment. Develop and implement individualized care plans based on patient needs and goals. Administer medications, treatments, and therapies as prescribed by physicians. Monitor patient progress and adjust care plans as necessary. Educate patients and their families about health management, treatment options, and post-discharge care. Maintain accurate and timely documentation of patient interactions and care provided. Collaborate with interdisciplinary teams to ensure continuity of care. Requirements Current Registered Nurse (RN) license in California. Minimum of 2 years of nursing experience in home health or a related field preferred. Basic Life Support (BLS) certification required. Strong communication and interpersonal skills. Additional Information Contract Duration: 13 weeks.
